Mounjaro injection site rotation: how to remember which side to inject

Guide - applies to Mounjaro, Wegovy, Ozempic, Zepbound and other weekly injections

Short answer: rotate between your stomach, thighs and upper arms, alternating left and right, and never inject the same spot two weeks running. The reliable way to remember which side you used is not memory. It is a written or logged record made at the moment you inject.

Why rotation matters

Injecting the same spot week after week can cause irritation, soreness and small lumps under the skin. Rotating sites gives each area time to recover. The patient leaflet for weekly GLP-1 injections asks you to change site each time for exactly this reason, and pharmacists give the same advice: alternate sides, and move around within each area rather than hitting one favourite spot.

A rotation pattern that is easy to keep

You do not need anything complicated. A pattern many people settle on:

  1. Week 1: left side of the stomach
  2. Week 2: right side of the stomach
  3. Week 3: left thigh
  4. Week 4: right thigh
  5. Then back to the start, shifting a few centimetres within each area every cycle

Upper arms work too if someone can help you reach, and within the stomach keep at least 5 cm away from your belly button. If a site is bruised, tender or lumpy, skip it for a cycle and let it recover. Your prescriber's instructions always come first.

"I'll remember where I injected" is a lie we all tell ourselves

Seven days is a long time. By the next jab day, most people genuinely cannot recall whether it was left stomach or right thigh. The common workarounds people use are notes apps, calendar entries, even dotting the site with a marker and taking a photo. They work until the one busy week you forget to write it down, and that is the week that matters.

How PepLevel handles it

When you log a dose in PepLevel, the injection site is part of the log: one tap on the site rotation grid, saved with the time and the amount. Next jab day, the grid dims the zones you used recently and marks the next site in your rotation, so you never second-guess. Because the site is captured at logging time, there is no separate note to remember, and your full site history is always one glance away.

Never second-guess the site

PepLevel logs the site with every dose and suggests where to go next. Coming soon to the App Store.
