Your first injection day: what to expect and how to prepare

Guide - applies to Mounjaro, Wegovy and other weekly weight loss injections

Short answer: the first jab is almost always easier than the anticipation. Most people feel nothing unusual on day one beyond possible mild redness at the site. Pick a day and time you can repeat every week, follow the pen instructions your prescriber and the leaflet gave you, and start your record from dose one; you will thank yourself at your first review.

Before the injection

  • Pick your day deliberately. You will inject on this day every week, so choose one where a dip in appetite or energy over the following couple of days is manageable. Many people choose a day when the next two days are quiet.
  • Read the leaflet, watch the official pen instructions. Your pharmacy, prescriber or the manufacturer's patient site will have step-by-step instructions for your specific pen. Follow those, not social media.
  • Choose a site. Stomach (at least 5 cm from the belly button), front of the thigh, or upper arm if someone can help. Clean, dry skin, no bruised or tender spots.

What day one usually feels like

Most people can carry on with a completely normal day: work, driving, everything as usual. The most common day-one experience is no experience, sometimes with slight redness, itching or a small bump at the injection site that settles on its own. Appetite changes often start within the first couple of days. If anything feels genuinely wrong, that is a question for your prescriber or pharmacist, not a search engine.

The first week

Side effects like nausea, when they happen, tend to appear a day or two after injecting and ease as the week goes on; they are most common when starting and after dose increases. Eating smaller meals and staying hydrated are the standard pieces of advice from prescribers. Everyone's pattern is different, which is exactly why your own record matters more than anyone else's timeline.

Start the record on day one

Your first review appointment will ask questions about weeks you will otherwise have to reconstruct from memory: when you injected, where, what you weighed at the start, what side effects came and when. Recording from dose one costs seconds and makes every later decision better informed:

  • Log the dose, the time and the site the moment you inject
  • Record your starting weight the same day
  • Note any side effects with the day they occur, so the pattern against your jab day is visible

How PepLevel handles it

PepLevel is built to start on day one: add your medication and your prescriber's plan, log the first dose in one tap with its injection site, and enter your starting weight. From then on, reminders handle the schedule, the rotation grid handles sites, and by your first review you will have a clean, complete record to show rather than a best guess.
