Side effects the day after your injection: why timing matters on a GLP-1
Short answer: Many people on Mounjaro, Zepbound, Wegovy or Ozempic notice side effects such as nausea, tiredness or low appetite most in the first one to three days after an injection, roughly when the medicine reaches its highest level. They are usually strongest in the weeks after starting or stepping up a dose and often ease over time. Logging what you feel, how bad it is and which dose it followed turns a vague bad week into a clear pattern you can discuss with your prescriber.
Why side effects follow a weekly rhythm
After a weekly injection, the amount of medicine in your blood climbs for a day or so, peaks, then slowly falls until the next dose. Tirzepatide (Mounjaro, Zepbound) reaches its highest level somewhere between about 8 hours and 3 days after an injection, and semaglutide (Wegovy, Ozempic) within about 1 to 3 days.
Stomach and appetite effects tend to track that peak. So a common pattern is: jab on Thursday, queasy and tired Friday and Saturday, back to normal by Monday, and a bit hungrier by Wednesday. Your own rhythm may be different, which is exactly why it is worth writing down.
The most common side effects
Across the patient leaflets for tirzepatide and semaglutide, the most reported effects are digestive:
- Nausea, sometimes with vomiting
- Diarrhoea or constipation
- Loss of appetite, indigestion, heartburn or burping
- Tiredness, headaches and dizziness
- Redness, itching or a small lump where you injected
In the trials these were most common while the dose was being increased, and for most people they were mild to moderate and faded over the following weeks.
What often helps
- Eat smaller meals and stop as soon as you feel full.
- Go easy on fatty, fried and very sweet foods, especially in the first days after a dose.
- Keep drinking. Nausea and diarrhoea make dehydration more likely.
- Plan around your worst day. Some people choose an injection day so the roughest day falls when they can rest.
When to get help
Contact your prescriber, pharmacist or NHS 111 if side effects are severe, stop you keeping fluids down, or do not ease. Get urgent help for severe stomach pain that will not go away (sometimes spreading to the back), yellowing skin or eyes, signs of a serious allergic reaction, or signs of severe dehydration such as confusion or passing very little urine.
How to record side effects so they are useful
"I felt rough a lot this month" is hard for a prescriber to act on. Four details make the record useful:
- What: the symptom, in plain words.
- How bad: a simple 1 to 5 scale is enough.
- When: the date, so it can be lined up with your doses.
- Which dose: the injection it followed, and at what strength.
After a few weeks this shows whether symptoms cluster on day one or day three, and whether they are fading at your current dose or getting worse after a step up. That is exactly what a prescriber wants to know before deciding whether to hold, continue or change your plan.
